Complete this form if your course is dependent upon COMPUTER RESOURCES at University College locations. 

You must complete a form for each course, for each semester.  New software installations require 10 weeks notice and proof of license.  For assistance completing this form, contact ITS at 800-696-HELP
Examples of Computer Resources.
  • Software on lab student-access computers.
  • Software on instructor computers (lab or broadcast classroom).
  • Software that came with a textbook.
  • Data files for students.
  • Web sites which require "plugins."
  • Any special computer hardware.
   You may assume that UC computers have.
